Role Summary

The Care Coordinator supports members in Enhanced Care Management (ECM) and other care coordination programs. This role serves as the main contact for low-risk members and focuses on outreach, appointment support, referral coordination, and navigation assistance.

Working with Registered Nurse Care Managers, Behavioral Health Care Managers, Community Health Workers, providers, and community partners, the Care Coordinator helps members stay connected to care and to the services they need. The role reports to the Care Coordination Manager and is part of an interdisciplinary care team.

Key Responsibilities

Member engagement and care coordination

  • Manage a caseload of low-risk Tier 3 members.
  • Complete phone-based outreach and follow-up aligned with each member’s care plan and program rules.
  • Help coordinate CHW visit routes for in-person visits as part of scheduling and care coordination support.
  • Develop trust with members through culturally responsive, relationship-based communication.
  • Support enrollment, onboarding, and ongoing engagement efforts.
  • Track member participation and help maintain involvement in care management programs.
  • Assist with appointment booking, reminders, and navigation through the care process.

Referrals and resource coordination

  • Arrange referrals for primary care, specialty care, behavioral health, and community services.
  • Monitor referral progress and follow up until completion when needed.
  • Support transportation planning, appointment logistics, and requests for durable medical equipment.
  • Connect members with social support services and community resources based on their needs.
  • Help maintain continuity of care across providers, health plans, and community organizations.

Team collaboration

  • Take part in weekly Systematic Case Reviews, team meetings, and case consultations.
  • Work closely with RN Care Managers, Behavioral Health Care Managers, Community Health Workers, and providers to support member goals.
  • Raise clinical concerns to licensed staff for review and action.
  • Carry out delegated non-clinical tasks that support the care plan.
  • Assist with care transitions, post-discharge follow-up, and coordination across settings.

Documentation and program operations

  • Record member contacts, referrals, and care coordination work accurately and on time.
  • Track outreach and engagement activity using internal systems and reporting tools.
  • Support Targeted Engagement List monitoring and member assignment workflows.
  • Follow ECM, health plan, and organizational documentation standards.
  • Contribute to quality improvement work and program audits.

Qualifications

Language and culture

  • Must be fluent in both English and Spanish, spoken and written.
  • Familiarity with Latino customs and cultural norms is preferred.

Education

  • High School Diploma or GED is required.
  • An Associate’s or Bachelor’s degree in Social Work, Public Health, Healthcare Administration, Human Services, or a similar field is preferred.

Experience

  • 1 to 3 years of experience in healthcare, care coordination, case management support, patient navigation, customer service, or community-based services.
  • Experience working with Medicaid, managed care, or underserved communities is preferred.
  • Background in appointment scheduling, referral management, and service coordination is preferred.
  • Familiarity with electronic health records and care management systems is preferred.
  • Experience documenting member interactions and keeping records accurate is required.
