About the Role
The Care Navigation Coordinator supports Care Management Operations by handling and routing incoming calls, solving straightforward patient issues, and directing people to the right department or resource to keep them moving through the process. The role also includes a range of operational projects and support tasks.
This position follows a Monday to Friday schedule that may vary, with current working hours of 12:30 PM to 9:00 PM ET.
Key Responsibilities
- Manage incoming calls for Care Management Operations and route them appropriately.
- Use strong judgment to provide quick, practical help for patients when possible.
- Direct callers to the correct team, process, or web resource when needed.
- Set up tele-consults and appointments with Medical Managers and the Behavioral Care Team.
- Prepare and fax records and documentation for regulated patients.
- Handle patient resupply for those who qualify.
- Complete assigned tasks and special projects efficiently while prioritizing correctly.
- Support staff with tools built using G Suite or Microsoft Office/Office 365.
- Answer customer questions and help resolve issues.
- Work collaboratively as part of the team.
- Communicate professionally by phone, video, email, and text.
Requirements
- Bachelor’s degree or an equivalent qualification, ideally in a health-related area.
- Background in customer service within a busy healthcare setting.
- Call center experience is considered an advantage.
- Strong critical-thinking ability.
- Careful attention to detail.
- Good understanding of basic system workflows and logic.
- Ability to read and follow standard operating procedures and work instructions in English, including HIPAA and system security policies.
- Professional written communication skills for email and other digital tools.
- Clear verbal communication for speaking with employees, vendors, patients, and other practices in person or by phone.
- Comfort working under pressure.
- Ability to understand medical terminology or internal workflow language when required.
Compensation
The role pays between USD 20 and 21 per hour.
Working Schedule
The position is remote within the United States and follows a weekday schedule. Current hours are 12:30 PM to 9:00 PM ET, though the schedule may vary.
