About Optimum Health Solutions

Optimum Health Solutions is an established allied health provider with more than 20 years of experience, 14 clinics across NSW and Tasmania, and a team of over 200 professionals. The organisation focuses on delivering evidence-based support that helps children, adults and families achieve positive, practical outcomes.

Role Overview

We are looking for an experienced Behaviour Support Practitioner to join a multidisciplinary team in Hobart. The role is centred on delivering Positive Behaviour Support services that improve quality of life, strengthen independence and support meaningful participation in the community.

Key Duties

  • Carry out behavioural support assessments.
  • Create detailed and effective Behaviour Support Plans.
  • Deliver evidence-based Positive Behaviour Support interventions.
  • Provide services across clinic, home, community and school settings.
  • Support children, adolescents and adults with complex needs.
  • Work closely with families, carers, support coordinators and other stakeholders.
  • Contribute to outcomes that enhance daily living and community involvement.

What We’re Looking For

The ideal applicant will bring current NDIS Behaviour Support Practitioner registration at Proficient level or above, along with prior experience in behaviour support or a closely related role. A sound understanding of Positive Behaviour Support principles, the ability to assess needs and develop plans, and strong communication and relationship-building skills are essential. You should also be confident working independently, contribute effectively within a multidisciplinary team, and bring a calm, patient and solution-oriented approach. A genuine commitment to supporting people with disability and their families is also important.

Why Join Us

This position offers access to structured career pathways, regular professional development, specialist mentoring and a collaborative team environment. You’ll work alongside occupational therapists, speech pathologists, physiotherapists, psychologists and other Behaviour Support Practitioners, with access to purpose-built therapy spaces and modern clinical resources. The role is designed for someone who wants to grow professionally while doing work that has a real impact.
