About the Role
At Apex Harmony Lodge, the focus goes well beyond medical care. This position is centered on seeing each resident as a whole person, guiding families through difficult stages, and keeping support well-coordinated, compassionate, and tailored to individual needs.
The organization is seeking a Case Manager who can connect people and services, handle complex situations calmly, and champion the well-being of individuals living with dementia.

Key Responsibilities
- Arrange and coordinate care plans so residents receive comprehensive, person-centred support.
- Work alongside nurses, therapists, doctors, and other specialists to deliver high-quality care.
- Develop trusted relationships with residents’ families and act as an important contact point.
- Support admissions, transitions in care, and discharge preparation.
- Track residents’ progress and help organize care reviews when needed.
- Contribute to audits, reporting, and improvement efforts connected to social care services.
- Keep records accurate and ensure practices align with the required standards.
- Take part in multidisciplinary case discussions and care-planning meetings.
- Support residents’ emotional, social, and psychosocial well-being by advocating for their needs.
Requirements
- A Degree or Diploma in Social Work, Psychology, or a closely related field.
- Background in dementia care, case management, healthcare, or community care is an added advantage.
- Strong verbal and written communication skills, along with good interpersonal ability.
- Capability to function effectively within a multidisciplinary setting.
- Comfort using Microsoft Office tools, including Excel, Word, and PowerPoint.
- Genuine commitment to supporting seniors and their families.
