Overview
Premera Blue Cross is hiring a Case Manager to join a team focused on improving members’ lives through coordinated, whole-person care. This role supports people experiencing behavioral health, medical, and substance use challenges by building trusting relationships, assessing needs, and guiding them toward practical goals that improve daily functioning and overall well-being.

Role impact
In this position, you will work alongside behavioral health clinicians, nurses, wellness specialists, members, caregivers, and providers to identify barriers, encourage behavior change, and help coordinate appropriate care. The work is centered on integrated case management, with an emphasis on meeting members where they are and helping them progress toward sustainable self-care.
Key duties
You will connect with target members, complete holistic assessments, identify social determinants of health and cultural factors, and help create individualized care plans. You will also review progress, update plans as needs change, support communication across the care team, and maintain documentation that aligns with accreditation, contractual, and legal standards.
Requirements and qualifications
This role requires an active Washington State or compact license in one of the accepted disciplines, along with at least 3 years of post-licensure direct patient or clinical care experience. A stable internet connection of at least 75 Mbps download speed is also required. Preferred qualifications include prior case management experience, CCM certification, CDP, CCP, LEAN training, and advanced motivational interviewing skills.
All case managers must hold a valid Washington State or MSL/compact license and meet Washington credentialing requirements. If you do not already have an active Certified Case Manager credential, you will need to obtain it within 24 months.
Training, schedule, and work setup
The role is telecommuter-based. Initial training lasts four to six weeks and runs Monday through Friday between 8:00 AM and 5:00 PM Pacific Time. After training, the position follows a 40-hour weekly schedule, including 5 hours of coverage for extended hours on weekdays between 5:00 PM and 7:00 PM or on Saturday between 9:00 AM and 2:00 PM Pacific Time. Schedules may change periodically based on member coverage needs.

Premera offers training and development support, including ongoing motivational interviewing instruction and help toward earning CCP, ICM, and CCM credentials. Compensation
Pay is based on factors such as location, market conditions, skills, and experience. The national salary range is $80,200 to $125,600, and the national plus salary range is $84,200 to $143,100 for higher-cost labor markets such as Western Washington and Alaska.
