About the Role
Join a home-health organization committed to improving patient outcomes through compassionate care delivered in the home. The company is Florida’s largest independent Medicare-certified home health provider and has spent more than 20 years building a reputation for patient-focused service.
What You’ll Do
- Visit referral partner offices with CCs, build working relationships, understand how each account prefers to receive referrals, and keep delivery preferences up to date.
- Go into the field on a regular basis to secure signatures for current and pending orders.
- Coordinate with referral partners to collect missing documentation when a CC is no longer with the agency or branch.
- Verify that physician details on profile pages are accurate and research, correct, and maintain any errors.
- Submit Smartsheet updates for changes to CC-assigned referral sources, including physicians, facilities, hospitals, and any other information that needs revision.
- Record daily documentation work with the date, action taken, and follow-up date, and review the previous week’s activity for pending items.
- Support order escalation efforts by coordinating with the home office, referral partners, and CCs.
- Attend meetings to share status updates on outstanding F2F items, hand-carry orders, escalations, and plans for collecting missing paperwork.
- Prepare and print orders for each CC and help map out a weekly signature-collection plan; when needed, send outstanding orders, F2F items, or hand-carry orders to CCs who are already in a referral source’s office and can obtain a signature during that visit.
- Run weekly F2F and order reports and work with CCs to make sure orders are completed within 14 days, or within the timeframe appropriate to the patient’s case.
- Review accounts that may need to be written off with RDS and decide whether they can be resolved or should be submitted for write-off.
- Maintain professional communication across departments to close out pending orders.
- Respond quickly to questions and issues, and work toward timely resolutions for concerns or discrepancies.
- Follow HIPAA requirements in all account-related work.
Requirements
- High school diploma or an equivalent credential is required.
- Prior experience using an EMR platform is required.
- Strong verbal and written communication skills, plus good organization and time management.
- Comfortable with advanced typing, Microsoft Office, and multitasking across several software tools while preserving data accuracy.
- Able to perform well in a deadline-focused setting.
- Capable of staying productive during heavy workloads and tight timelines.
