Role overview
RADIAS Health is looking to add 2 full-time Case Managers to its Targeted Case Management team in Ramsey County. This position focuses on supporting adults living with serious and persistent mental illness, including those with co-occurring substance use disorders, through person-centered services that strengthen recovery, community connection, and day-to-day stability.
The goal of the role is to help clients reach and maintain the best possible quality of life based on their individual needs and preferences. In this position, you will connect clients with medical, social, educational, vocational, financial, housing, legal, and other essential supports tied to their mental health care.
Work schedule and location
This is a hybrid role, with some work carried out in the community and other tasks completed from home. Standard hours are Monday through Friday, either 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM, for 40 hours per week.
Compensation
The starting pay range for this opening is $52,797 to $54,465, plus an office benefit package covering parking or remote expenses at $130 per month. Key duties
The Case Manager is responsible for delivering coordinated, recovery-focused support services and maintaining compliance with applicable Minnesota requirements.
