Role overview

This opportunity is for a Clinical Pharmacist to join the team at Brooklands Medical Practice. The post sits within an established pharmacy team and focuses on supporting medicines optimisation across the practice. You will work alongside a broad multidisciplinary group that includes GPs, nurses, physiotherapists, mental health practitioners, physician associates and the wellbeing team. The organisation’s approach is centred on improving local communities by removing barriers and keeping patient wellbeing at the core.

Key duties

  • Handle medication-related requests and questions from patients, and keep track of medicines use as part of long-term condition care.
  • Review clinical correspondence and hospital discharge information, then act on appropriate medicine changes and support patients in optimising their treatment.
  • Carry out clinical medicines reviews, especially for people with polypharmacy, long-term conditions, multiple conditions, frailty or more complex needs.
  • Work with the prescribing lead and the wider ICB medicines management team to promote evidence-based, cost-conscious prescribing aligned with current guidance and safety alerts.

Essential criteria

  • You must be a registered pharmacist with at least two years of post-registration clinical experience.
  • Membership of the General Pharmaceutical Council or the Royal Pharmaceutical Society is required.
  • You should have a solid understanding of general practice and primary care prescribing.
  • Strong knowledge of evidence-based healthcare principles and their practical application is needed.
  • Excellent verbal and written communication skills are essential for working with varied audiences.
  • You must be able to engage with the public discreetly and sensitively, adapting communication style to individual needs.
  • Good organisational ability is important.
  • A clear commitment to high-quality, responsive and flexible patient care is expected, along with willingness to take part in teaching.
  • You should show commitment to ongoing professional and personal development through recent learning or study.
  • Good IT and keyboard skills are required.

Desirable experience

  • Experience working with deprived and ethnically diverse populations.
  • Recent experience as a pharmacist in primary care.
  • Non-medical prescriber status.
  • Completion of CPPE or PCPEP.
  • A postgraduate diploma in clinical pharmacy, or an equivalent qualification.
  • A full, valid UK driving licence, business car insurance and daily access to a car.

Working pattern and contract details

This is a part-time role of 30 hours per week, based mainly at Brooklands Medical Practice. Hours will be arranged within Monday to Friday, 08:30 to 18:30. The salary is pro-rated from £47,810 to £54,710 per year, depending on experience.
