Role Overview
Chronic Care Staffing is looking for a proactive, resourceful Follow-up Coordinator who can help patients move through the referral process and connect with the right specialists. In this remote role, you will monitor referrals after they are submitted, reach out to specialist offices, work through scheduling issues, and keep patients informed about what comes next in their care.
This position suits someone who enjoys solving problems, handling coordination work by phone, and contributing to better patient outcomes.
Key Responsibilities
- Keep an eye on patient referrals after submission and track their progress.
- Contact specialist practices by phone, fax, and secure online systems to confirm referral status.
- Chase outstanding referrals and appointment requests until they are resolved.
- Look into delays and obstacles that are preventing scheduling from moving forward.
- Find alternate providers when a specialist is not available or cannot take a patient.
- Share updates and next steps with patients when needed.
- Use internal reference materials to identify suitable specialists.
- Record details accurately in internal systems and maintain organized documentation.
- Work closely with Scheduling Coordinators and other teammates to keep operations running smoothly.
- Learn additional coordination tasks so you can support the team across different needs.
Requirements
- Prior background in customer service, call center work, administrative support, medical reception, or healthcare coordination.
- Strong phone etiquette and clear verbal communication skills.
- Good organization, time management, and multitasking ability.
- Comfort working on your own and handling follow-up tasks with minimal oversight.
- Strong troubleshooting mindset and close attention to detail.
- Willingness to learn new tools, systems, and workflows.
- Stable internet connection and the ability to work from home.
- Helpful experience in a medical office or other healthcare setting.
- Exposure to referral coordination or appointment scheduling is a plus.
- Experience speaking with doctors, clinics, or other healthcare providers is beneficial.
- Awareness of scheduling issues related to insurance is advantageous.
- Understanding of healthcare administrative processes is preferred.

Compensation
The role pays between $16.00 and $17.00 per hour.
