Job Description

The Healthcare Assistant will be responsible for delivering daily clinical services to clients under the Home Care Services (HCS) program. This role requires professional conduct at all times, ensuring safe and timely care delivery to clients and accountability to the organization.

Job Responsibilities

• Provide direct care to clients in alignment with the Community Care Skills Standards Framework.
• Execute basic clinical tasks including vital signs monitoring, assisting with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (iADL).
• Administer medication reminders or assistance.
• Escort clients and assist with transfers and the use of mobility aids.
• Conduct appropriate exercises and activities for clients.
• Ensure service delivery adheres to contractual agreements and maintain accurate documentation.
• Contribute positively to the team, fostering a motivated and growth-oriented environment.
• Engage in outreach and related activities.
• Communicate effectively with colleagues and clients.
• Undertake any other duties as assigned by supervisors or management.

Qualification & Skills

• Possession of a Basic Nursing Aide Certificate is required.

Other Information

• Relevant experience of at least two years is necessary, with a preference for experience in community nursing.
• Key competencies include being people-oriented with strong interpersonal skills, the ability to manage multiple tasks in a fast-paced setting, good computer proficiency, meticulous attention to detail, basic English communication, a committed and positive attitude, and strong teamwork capabilities.
