Role overview

This is a patient-facing coordination position in a home health setting. The intake coordinator is responsible for handling referrals promptly and accurately, making sure patient information is captured correctly and routed to the right internal teams. The role supports access to care by keeping intake workflows organized, maintaining data quality, and working closely with referral partners, clinical staff, and intake specialists.

The position is based in the United States and is listed on behalf of a partner organization, which will handle applications and the next steps. Weekend coverage is required, and the role suits someone who is comfortable in a structured, fast-moving healthcare environment.

What you'll do

  • Accept, evaluate, and enter home health referrals with speed and accuracy, ensuring patient and clinical details are recorded correctly in the system.
  • Apply basic diagnosis coding from referral paperwork and keep intake records clean and accurate.
  • Manage intake processes using standardized communication methods, including scripted welcome calls and set procedures.
  • Work with clinical intake staff to assess whether a referral is appropriate and direct cases to the correct path.
  • Communicate with outside referral sources and internal stakeholders to keep information flowing and care moving forward.
  • Help complete patient onboarding on time while following organizational requirements and intake standards.
  • Stay organized and responsive while handling a large number of time-sensitive intake requests.

Requirements

  • Hands-on experience with Homecare Homebase (HCHB) is mandatory.
  • An associate degree is preferred.
  • At least 1 year of experience in home health or another related healthcare setting is preferred.
  • At least 1 year of experience in customer service or a patient-facing support role.
  • Strong attention to detail and the ability to manage clinical and administrative data accurately.
  • Clear communication skills for working with patients, referral sources, and clinical teams.
  • Comfort working in a structured, fast-paced, deadline-driven environment.
  • Solid organization and multitasking ability when dealing with high intake volumes.
  • Availability to work weekends, with Saturday and Sunday coverage required.
  • Willingness to work either a 5x8 or 4x10 schedule.

Compensation
