Role overview

Included Health is building a coordinated care model designed to help members receive smoother, more connected support across both administrative and clinical needs. This clinician-led program serves members enrolled in All Included Care (AIC), Virtual Primary Care (VPC), and the Connected Care Program (CCP), with a focus on anticipating needs and creating a seamless experience.

As a Connected Care Assistant, you will partner with the clinical team to support members living with complex or chronic conditions. The role centers on care coordination, member support, and carrying out non-clinical services identified by the care team. This position requires a compassionate approach, familiarity with healthcare workflows, and strong service skills. Connected Care Assistants are not permitted to give medical advice, make diagnoses, perform evaluations, or interpret clinical information for members.

Key duties

Your work will involve member engagement through scheduled shifts that may include calls, chats, internal referrals, and targeted outreach to encourage eligible or at-risk members to join CCP. You will also use autodialer tools, manage care-plan task queues, coordinate care for members and their caregivers as directed by the clinical team, and help navigate employer-sponsored benefits.

In addition, you may open related service cases such as Provider Match, Concierge Referrals, Expert Medical Opinion, and Claims when requested by the clinical team. Administrative work will include drafting letters and mailings, requesting medical releases and records, handling callback tickets, referrals, and escalations, and monitoring email-based referrals and communications. You may also work within systems and feeds such as Ovia and ACM insurance feeds, attend huddles, join training sessions, and follow organizational policies as well as applicable healthcare regulations.
