Role overview

Proforma Health is looking for a Medical Scribe to work alongside primary care providers in a live clinical setting. This position focuses on accurate, real-time charting of patient visits and support for day-to-day care team operations at the Cleveland, OH location.

Core duties

You will capture encounter details as they happen, including the patient history, provider assessment, treatment plan, and physical examination notes. The role also includes helping with coding, visit summaries, documentation quality, and routine administrative coordination.

Responsibilities

• Document patient visits in real time during primary care encounters.
• Record key chart elements such as history, assessment, plan, and physical exam findings.
• Assign CPT and ICD-10 codes as part of the documentation workflow.
• Prepare after-visit summaries for patients and care teams.
• Support clinical documentation improvement by reviewing records and using population health tools.
• Assist with administrative tasks, including placing orders and referrals.
• Help address assigned tasks for the care team to keep workflow moving efficiently.

Skills and qualifications

Success in this role requires familiarity with medical terminology, strong verbal and written communication, comfort using computers, and the ability to adapt quickly to changing clinical workflows. Fast typing is important, and candidates with multilingual ability are preferred.
