The company supports palliative and hospice care for people with serious illness who are at high risk of receiving overly aggressive medical treatment. This is an underserved area in today’s healthcare market.
Empassion works with payers and other risk-bearing organizations, partnering with community-based palliative and hospice providers to deliver stronger outcomes. The company is already operating in 44 states and continues to expand quickly.
Although the role is fully remote, the organization works in a hybrid remote/in-person style overall and is headquartered in New York City. The team values asynchronous communication, while also staying ready to jump on Zoom or a call whenever needed.
Role Overview
The care coordinator will work directly with patients, healthcare providers, and families within Empassion’s palliative care program. In this role, you will help patients join the program, coordinate with their providers, make sure they receive excellent care, and serve as a dependable point of contact for them and their loved ones.
This position is part of the Central Operations team, which is made up of care coordinators and led by a manager with substantial experience in healthcare contact centers and care coordination. Empassion provides detailed training and resources to support success in the role. Team communication happens regularly through Zoom and Slack.
What Success Looks Like
- Explaining the value of palliative care to patients in a clear and reassuring way
- Helping patients enroll in the program and connecting them with palliative care providers
- Responding to questions from patients, caregivers, and providers about palliative care and the broader program
- Recording patient interactions accurately and routing patient concerns to the appropriate care team
- Following up with patients to confirm they are receiving high-quality support through Empassion’s provider network
- Working with the team to improve internal processes
What You Will Bring
- At least 2 years of relevant experience in customer service, a call center, or healthcare
- A working understanding of healthcare
- A compassionate, people-first mindset; experience in social work or another human-centered field is preferred
- Genuine motivation to create a better experience for patients and caregivers
- A service-oriented attitude and willingness to go above and beyond for patients
- Strong empathy and communication skills, especially over the phone
- Comfort using technology and working in a fast-moving startup environment
- Spanish-English bilingual ability is helpful but not mandatory
- Reliable secure Wi-Fi access
