Role Overview
The organization is looking for an HCC Coder/QA professional for its Chennai or Hyderabad location. The role involves reviewing clinical records, assigning diagnosis codes, supporting coding accuracy, and maintaining compliance standards.
Experience and Location
This opportunity is open to candidates with more than 1 year of experience. The acceptable range mentioned is 1.5 to 4 years. The work location can be Chennai or Hyderabad, and notice-period candidates are acceptable.
Responsibilities
- Examine medical charts and clinical documentation to determine diagnoses and treatments.
- Apply ICD-10-CM diagnosis codes appropriately to patient records.
- Translate diagnoses into Hierarchical Condition Categories (HCC).
- Verify that coding is applied accurately and consistently across records.
- Review documentation and audit records to check for completeness and correctness.
- Share coding-related feedback with physicians and other providers to improve documentation quality.
- Support training for physicians, providers, and clinic teams on documentation compliance expectations.
Requirements
- Solid practical knowledge of medical coding.
- Strong focus on precision and detail.
- Commitment to compliance and accurate coding practices.
- Working familiarity with ICD-10-CM, CPT, HCPCS, and HEDIS CAT II codes.
- Any one of the following certifications is required: CRC, CPC, or CCS.
