About Habitat Health

Habitat Health helps older adults enjoy more healthy days in their own homes and neighborhoods. Using the Program of All-Inclusive Care for the Elderly (PACE), the organization delivers coordinated medical and social support, including care in centers, at home, meals, transportation, and help with daily living. Its model is designed to be affordable, scalable, and centered on helping participants live on their own terms.

The company is backed by major healthcare organizations and investors, including New Enterprise Associates, Kaiser Permanente, and Town Hall Ventures, and is growing its team to support expanded care for aging populations.

Role summary

The Quality Improvement Analyst will strengthen quality infrastructure across PACE centers by managing core data systems, supporting participant safety and regulatory reporting, coordinating quality committee work, and partnering with center and central teams to identify patterns, close gaps, and improve outcomes.

Estimated start date: 8/10/2026.

What you'll do

  • Keep quality dashboards and CMS/state reporting tools up to date, and surface missing metrics, trends, and follow-up actions.
  • Build monthly metric summaries and action lists so center teams can stay aligned with quality targets.
  • Help prepare quarterly submissions, regulatory review materials, and case-level documentation in coordination with Compliance.
  • Monitor participant safety events, assist with investigations and root-cause analysis, and prepare updates for center and leadership teams.
  • Check that safety incidents are recorded, escalated, and documented correctly, and loop in Compliance when issues need attention.
  • Review emergency department visits and inpatient admissions to track utilization and quality performance across centers.
  • Spot utilization patterns and share practical recommendations with center teams and leadership.
  • Support Quality Improvement Committee operations, including scheduling, agenda building, data presentations, strategic recommendations, and process improvements.
  • Assist with the annual QIC review and governing body approval for each center’s Quality Improvement Program.
  • Conduct recurring internal quality audits and maintain records that support regulatory readiness.
  • Escalate audit findings that require Compliance involvement, corrective action plans, or support for external audits.

What we're looking for

  • At least 2 years of experience in quality improvement, healthcare operations, compliance, or a similar area.
  • Strong organization and dependable follow-through across recurring tasks, deadlines, and cross-functional work.
  • Comfort working with data, including maintaining trackers, identifying trends, and turning findings into clear recommendations.
  • Confident written and verbal communication skills, with the ability to present findings and facilitate meetings.
  • Ability to use Excel or similar tools for tracking, analysis, and reporting.
  • Alignment with Habitat Health’s purpose and values, and enthusiasm for putting them into practice every day.
  • Ability to work well in a fast-moving environment with ambiguity, change, and continuous improvement.
  • Growth mindset, curiosity, receptiveness to feedback, and willingness to use data to guide decisions.
  • Integrity, accountability, and a strong sense of ownership over work and organizational impact.
  • Clinical experience is helpful but not required.
  • Background in PACE, Medicare/Medicaid managed care, or programs serving older adults with complex needs is preferred.
  • Familiarity with PACE regulations, DHCS requirements, HPMS reporting, or quality/compliance committee operations is preferred.

Work environment and physical expectations

This role may involve working in healthcare, administrative, community, or remote settings depending on business needs. The position may require extended periods of sitting or moving around, regular communication with participants, caregivers, teammates, and other stakeholders, use of office or clinical equipment, travel between sites or community locations, and adherence to safety procedures, infection control rules, and PPE requirements when applicable.

The anticipated base pay range is $81,000 to $107,000 per year. The role is also eligible for a target annual bonus equal to 10% of base salary and may qualify for additional incentive compensation, depending on applicability.

Final compensation will be based on factors such as qualifications, skills, experience, education, certifications, location, and business needs.

Benefits include medical, dental, and vision coverage; paid parental leave; short- and long-term disability; life insurance; flexible spending accounts; a 401(k) plan with company match; paid time off; and company-paid holidays.

Employment terms

Employment is at-will, meaning either the employee or the company may end the relationship at any time, with or without cause or notice, subject to applicable law. Nothing in this posting creates a contract of employment or changes the at-will nature of employment. Any statement to the contrary must be in writing and signed by an authorized company representative.

Because the role is remote, employment will follow the laws that apply to the employee’s work location, along with all relevant legal requirements.

Vaccination and infection control

Habitat Health follows vaccination and infection control policies for certain roles and work locations. For COVID-19 and influenza, team members working in a participant care setting must provide proof of vaccination or a signed declination form and follow masking requirements in care settings, where permitted by law.

Reasonable accommodation requests based on disability, medical condition, or sincerely held religious belief, practice, or observance will be considered in accordance with applicable law. As a condition of employment, team members must comply with all applicable infection control procedures, PPE requirements, and vaccination requirements tied to the role and location, as allowed by law.

Equal opportunity and inclusion

Habitat Health is an equal opportunity employer committed to inclusion. Hiring decisions are based on qualifications, merit, and business needs. The company does not discriminate based on race, color, religion, creed, sex, gender, pregnancy, childbirth, breastfeeding, related medical conditions, gender identity, gender expression, sexual orientation, marital status, domestic partner status, age, national origin, ancestry, citizenship, military or veteran status, disability, medical condition, genetic information, reproductive health decisions, or any other legally protected characteristic.

Qualified individuals with disabilities and individuals needing religious accommodation are supported in line with applicable law. If you need a reasonable accommodation during the application or interview process, to perform essential job duties, or to access employment benefits and privileges, contact [email protected].

Habitat Health encourages candidates from all backgrounds to apply, even if their experience does not match every listed qualification.

Employment verification

Habitat Health uses E-Verify and will provide the federal government with information from Form I-9 to confirm work authorization in the United States. If E-Verify is unable to confirm authorization, the company will provide written instructions and an opportunity to contact DHS and/or SSA before any adverse action, including termination, is taken. E-Verify is used only after an offer is accepted and Form I-9 is completed, to the extent allowed by law.
